    PARTNERSHIP AGREEMENT FOR USE OF CHILDREN AND YOUTH GRIEF NETWORK RESOURCES T H I S A G R E E M E N T is made in duplicate on the (Date, Month, Year) B E T W E E N:

    HeartHouse Hospice INC (Hereinafter referred to as "HHH") as the Lead Organization for the Children and Youth Grief Network (CYGN) And ORGANIZATION IN CONSIDERATION of the following covenants and other good and valuable consideration the Parties hereto agree as follows:
    The resource and its content are the property of the CYGN/HHH and are to be represented as such.

    2. Name of org. has been granted permission to use the resource to promote the mission, vision and work of the CYGN.

    3. There is no cost for this product or its use with the exception of any costs incurred to print/ship the resource.

    4. Any costs that will be incurred by either party will be clearly identified and confirmed prior to the exchange.

    5. If the CYGN/HHH prints and provides copies of postcard, name of org., agrees to pay the costs incurred for the work prior to receipt of the resource.

    6. Name of org. understands and agrees that to protect the integrity of the resource and the CYGN, that the written content will not be modified or change without written permission from both the CYGN and HHH.

    7. The CYGN logo must not be altered, moved or changed and is to remain on the resource in any format including but not limited to digital, online or print.

    8. Name of org. agrees to report once a year to the CYGN/HHH.

    9. The CYGN/HHH agrees that name of org can add its logo and the logo of collaborative partners that work with individuals who are seriously ill, dying or have died and/or provide support to grieving children, caregivers or families.

    10. The inclusion of additional logos is for the sole purpose of directing individuals and families to services/support that can further assist them with a grief or death. Any logos not connected to service or support organizations are not permitted to be included.

    11. This Agreement constitutes the entire Agreement between the parties hereto with respect to the subject matter here of, and supersedes any or all prior negotiations, understandings, undertakings, warranties, representations, or agreements of whatsoever nature. This Agreement may not be varied, modified or amended, except by an instrument duly executed by both parties hereof.

    Canadian Children's Grief Symposium

    Join us for Canada's first national Children's Grief Symposium. Participants will have the opportunity to learn foundational concepts,
    practical tools and innovative methods that will help them to support children, youth and families who are coping with grief and bereavement
    in their own communities. Workshops will be delivered by practitioners and educators from across Canada who have extensive experience
    and knowledge in children's grief and bereavement care.
